Telehealth Nurse, PHRN, Manila (Registered Nurse)
Provide remote clinical support to patients on a multidisciplinary weight-management program. Work within the medical support team to triage, assess, and coordinate patient care across clinicians and pharmacies.
What you'll do
- Triage and assess patient medical enquiries via email
- Respond promptly with accurate medical information and escalate as needed
- Monitor trackers and proactively reach out to support adherence
- Coordinate GP and pharmacy communications for patient care
- Conduct 4-month and 8-month patient reviews under Sydney guidance
